How healthy does Concord think it is? The town’s trying to find out through a Community Health Needs Assessment.
The survey will be used to set goals for town-specific planning, including understanding the health concerns among residents and developing programs to address those concerns; improving health services; and connecting organizations and individuals to promote a healthier community.

Paper copies are also available at the town’s Health Division, 141 Keyes Road, and at the Council on Aging, 1276 Main Street.
“Being a healthy town is about more than delivering quality health care to residents,” the introduction to the survey says.
“Where you live, learn, work, and play all have an enormous impact on your health. Understanding our community’s current health status — and all of the factors that influence health — is important for identifying future priorities, existing strengths and assets upon which to build, and areas for further collaborative efforts.”
The survey is for residents 18 and older and takes approximately 10 to 15 minutes. It’s anonymous and contains questions on sensitive subjects, such as mental health and substance use. Partial responses cannot be counted.
Resources will be provided at the end of the survey.
